Determination:
The Implant placement, Custom abutment, and Abutment supported porcelain fuse to metal crown (noble metal) are medically necessary.

The patient is missing a tooth. There is a tooth on top of the tooth, so with a replacement tooth in this area, there will be chewing function restored. An implant is the most conservative way to replace this tooth. A bridge would require the drilling down of two teeth that do not need to be treated. There is no other tooth in the lower jaw that needs replacement. The implant, abutment and crown are medically necessary to restore this area.
