There is no documented reason as to why the patient cannot be treated with the preferred formulary agents. The medical records do not support and adequate trial or contraindication to any of the preferred agents. There are no high quality clinical trials or standard of care guidelines supporting the preferential use or superiority of Aimovig relative to the preferred agents, Metoprolol, Timolol, Nadolol, Atenolol, Divalproex Delayed-Release or Extended-Release tablet, Amitriptyline, Nortriptyline, and Venlafaxine ER. Therefore, the treatment with Aimovig is not medically necessary.
